Patient & Office Policies
Appointments & Arrival
Please arrive on time for your scheduled appointment and allow additional time for registration when requested. Patients arriving more than 15 minutes late may need to be rescheduled to ensure adequate time for safe, thorough care.
Cancellation & No-Show Policy
We require at least 48 hours' notice when canceling or rescheduling an appointment. Late cancellations and missed appointments may be subject to a $25 fee. Scheduled surgeries canceled or rescheduled with less than 48 hours' notice, or missed without notice, may be subject to a $100 fee. Exceptions may be made at the discretion of practice management for emergencies or other extenuating circumstances.
Payment & Insurance
Copays, deductibles, coinsurance, outstanding patient balances, and other amounts determined to be the patient's responsibility are due according to the practice's financial policies. Patients are responsible for providing current insurance information and notifying our office of changes in coverage.
Card on File
FBL Physicians may require a valid payment method to be securely maintained on file for scheduling and payment purposes. When applicable, patients will be provided the terms of the charge and authorization. Payment information is processed through our approved secure payment system.
Forms & Paperwork
FMLA, disability, leave, insurance, and other forms requiring completion by our office are subject to a $25 paperwork fee. Please allow up to 72 business hours for completion.
Medical Records
Patients may request copies of their medical records in accordance with applicable law and practice procedures. Requests are processed within the timeframes required by law. Any applicable fees will be communicated before processing.
Respectful & Safe Environment
We are committed to providing a safe, respectful environment for our patients, visitors, providers, and staff. Abusive, threatening, discriminatory, sexually inappropriate, or disruptive behavior toward staff, providers, patients, or visitors will not be tolerated and may result in removal from the premises or dismissal from the practice when appropriate.
Communication & Patient Portal
Patients are encouraged to use the patient portal for non-urgent communication with the care team. Portal messages are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or seek emergency medical care.
