Medical Records
Copies of medical records prepared in our office are $1.00 per page. Payment is required before records are released. A valid authorization must be completed before protected health information can be provided.
FMLA and Other Paperwork
A $25 administrative fee applies to FMLA, disability, leave, insurance, school, employment, and other forms requiring provider completion. Payment is required before completed paperwork is released. Patients must complete their portion and submit all required pages before processing can begin.
If you are experiencing a medical emergency
Call 911 or go to the nearest emergency department. Website forms, appointment requests, emails, and patient portal messages are not monitored continuously and should not be used for emergencies.
For urgent pregnancy-related or postoperative concerns, follow the specific instructions provided by your healthcare team or call the office for the appropriate after-hours process.
