We look forward to caring for you. New patients should arrive early enough to complete registration and allow our team to verify insurance and appointment information.
Before your first visit:
• Complete all required patient forms.
• Bring a current photo ID.
• Bring your insurance card.
• Bring a current list of medications and allergies.
• Bring relevant medical records, imaging reports, or laboratory results.
• Bring your referral or authorization information when required.
• Be prepared to pay any applicable copayment, deductible, or outstanding balance.
Insurance verification does not guarantee payment. Patients are responsible for understanding their individual benefits and financial responsibility.
Your health information, securely available.
Use the patient portal to review available health information, send nonurgent messages, view certain results, and access other features provided by the practice’s portal system.
The patient portal is not monitored continuously and should not be used for medical emergencies or urgent concerns.
Insurance and payment options.
FBL Physicians works with a variety of insurance plans. Coverage, benefits, referrals, prior authorizations, deductibles, and copayments vary by plan.
Prescription refill requests.
Please request routine prescription refills before your medication runs out. Refill requests may require provider review, an appointment, laboratory testing, or other follow-up before approval.
• Contact your pharmacy first to confirm whether refills remain.
• Submit nonurgent refill requests through the approved patient portal or office process.
• Include the medication name, dose, pharmacy name, and pharmacy telephone number.
• Allow appropriate business time for review.
• Some medications cannot be refilled without an appointment.
• Controlled medications may have additional requirements.
Submitting a refill request does not guarantee that the prescription will be approved or renewed.
Requesting your medical records.
A completed authorization is required before protected medical information can be released, except when otherwise permitted by law.
1. Complete the Authorization for Release of Medical Records.
2. Identify where the records should be sent.
3. Submit the signed form through an approved secure method.
4. Allow the legally permitted processing period.
5. Pay any applicable records fee when allowed.
Please Note: Copies of medical records prepared in our office are $1.00 per page. Payment is required before records are released. A valid authorization must be completed before protected health information can be provided. Additional requirements may apply depending on how and where the records are sent.
Forms requiring provider completion.
FMLA, disability, leave, insurance, school, employment, and other third-party forms require administrative and provider review. Completion times and fees may vary depending on the form and the information required.
• Complete the patient or employee section before submission.
• Include all required claim numbers, employer information, and return instructions.
• Submit complete and legible forms.
• Allow the practice’s stated processing time.
• Confirm that the organization has received the completed documents.
A $25 administrative fee applies to FMLA, disability, leave, insurance, school, employment, and other forms requiring provider completion. Payment is required before completed paperwork is released. Patients must complete their portion and submit all required pages before processing can begin.
